Essen Health Care's Health Home program is a New York State Medicaid program designed for individuals with multiple chronic conditions or a single qualifying condition such as HIV/AIDS or a serious mental illness. As a designated Health Home, we provide intensive care management that coordinates all aspects of a patient's care, including medical, behavioral health, and social services. Our care managers develop personalized care plans, monitor progress, and ensure patients are connected to all available resources.
What We Provide
- Comprehensive care assessment and planning
- Coordination across all healthcare providers
- Health promotion and self-management education
- Referrals to community and social support services
- Transition of care management (hospital to home)
- Chronic disease monitoring and follow-up
Eligibility
- Medicaid recipient
- Two or more chronic conditions (e.g., diabetes, heart disease, asthma, substance use disorder)
- OR one qualifying condition: HIV/AIDS, serious mental illness, or substance use disorder
