How to Request Your Records
Under HIPAA, you have the right to access your medical records. Essen Health Care is committed to fulfilling record requests promptly and securely.
Submit a Written Request
Complete a Medical Records Release Authorization form. You may obtain the form at any Essen Health Care location or by contacting our Medical Records department.
Provide Identification
A valid photo ID is required to verify your identity. Requests for another person’s records must include a signed authorization from the patient or legal guardian.
Processing Time
Requests are typically processed within 10–15 business days. Urgent or time-sensitive requests can be expedited — please notify our staff when submitting.
Receive Your Records
Records can be mailed, faxed to your new provider, or made available for pick-up at the location where you received care.
You can also access recent lab results, visit summaries, and other health information through the Essen Health Care Patient Portal.
