The program aims to improve quality of life, reduce strain on caregivers, and enable people with dementia to remain in their homes and communities.

Eligibility
- Diagnosis of dementia (Alzheimer's or related dementias)
- Medicare beneficiary
- Living in the community (not in a nursing facility)
- Have an unpaid caregiver willing to participate
