In 2024, Essen Health Care launched Atlas IPA, an Independent Practice Association designed to bring value-based care to the communities we serve. Through the ACO REACH program with the Centers for Medicare & Medicaid Services, Atlas IPA aligns financial incentives with patient outcomes rather than the volume of services provided.
Value-based care represents a fundamental shift in how healthcare is delivered and paid for. Instead of rewarding doctors for ordering more tests and procedures, this model incentivizes keeping patients healthy through preventive care, chronic disease management, and care coordination. When patients are healthier, everyone benefits—patients enjoy better quality of life, providers can focus on meaningful care, and healthcare costs are more sustainable.
Atlas IPA brings together over 400 physicians across Essen's network, providing the infrastructure for data sharing, quality measurement, and coordinated care delivery. Our care managers work with high-risk patients to ensure they have access to the services they need, from medication management to social services.
