The GUIDE (Guiding an Improved Dementia Experience) Model, a new initiative from the Centers for Medicare & Medicaid Services, aims to change that by providing comprehensive, coordinated care for people living with dementia and their caregivers.

Through GUIDE, eligible patients receive a dedicated care team that includes a physician, care navigator, and caregiver support specialist. The team develops a personalized care plan that addresses not just medical needs but also safety, daily functioning, behavioral health, and caregiver well-being.
One of the most impactful features of the GUIDE Model is respite care, which gives family caregivers temporary relief from their caregiving responsibilities. Caregiver burnout is a serious concern—studies show that dementia caregivers experience higher rates of depression, anxiety, and physical health problems compared to non-caregivers.
