Managing Diabetes in the Bronx: A Community Approach

CommunityJanuary 8, 20257 min readDr. Sumir Sahgal

Diabetes is a major health challenge in the Bronx, where rates are significantly higher than the national average. At Essen Health Care, we believe that managing diabetes requires more than just medication—it takes a community approach that addresses nutrition, exercise, education, and access to consistent medical care.

Our diabetes management program connects patients with primary care physicians, endocrinologists, nutritionists, and care coordinators who work together to create comprehensive treatment plans. We focus on A1C monitoring, medication management, and lifestyle counseling to help patients take control of their health.

One of the biggest barriers to diabetes management in our community is access to healthy food options. Many neighborhoods in the Bronx are classified as food deserts, making it difficult for patients to follow dietary recommendations. Our care coordinators help connect patients with local resources, including farmers' markets, food assistance programs, and cooking classes that teach affordable, diabetes-friendly meal preparation.

Early detection is critical. We recommend that all adults over 45—and younger adults with risk factors such as obesity, family history, or a sedentary lifestyle—get screened regularly. If you or a loved one is living with diabetes or pre-diabetes, schedule a visit at any of our 40+ locations to start your journey toward better health.
